IntroductionDeep brain stimulation has become a standard treatment option for select patients with Parkinson’s disease. The selection process and surgical procedures employed have, to date, not been standardized.MethodsA comprehensive 58-question web-based survey was developed with a focus on DBS referral practices and peri-operative management. The survey was distributed to the Parkinson’s Foundation Centers of Excellence, members of the International Parkinson’s Disease and Movement Disorders Society, and the Parkinson Study Group between December 2015 and May 2016.ResultsThere were 207 individual respondents drawn (...) from 59 countries and 6 continents, of whom 64% received formal training in DBS. Thirteen percent of centers reported that DBS could proceed despite a confidence level of < 50% for PD diagnosis. A case-based approach to DBS candidacy was applied in 51.3% of centers without a cut-off for levodopa-responsiveness. Surprisingly, 33% of centers regularly used imaging for diagnostic confirmation of idiopathic PD. Thirty-one percent of centers reported that neuropsychological evaluation did not affect DBS target selection. Approximately half of the respondents reported determination of DBS candidacy based on a multidisciplinary committee evaluation and 1/3rd reported that a committee was used for target selection. Eight percent of respondents felt that psychosocial factors should not impact DBS candidacy nor site selection. Involvement of allied health professionals in the preoperative process was sparse. There was high variability in preoperative education about DBS outcome expectations. Approximately half of the respondents did not utilize a “default brain target,” though STN was used more commonly than GPi. Specific DBS procedure techniques applied, as well as follow-up timelines, were highly variable.ConclusionResults revealed high variability on the best approaches for DBS candidate selection, brain target selection, procedure type, and postoperative practices. Cognitive and mood assessments were underutilized. There was low reliance on multidisciplinary teams or psychosocial factors to impact the decision-making process. There were small but significant differences in practice across global regions, especially regarding multidisciplinary teams. The wide variability of responses across multiple facets of DBS care highlights the need for prospective studies to inform evidence-based guidelines. (shrink)

Hegel introduced the Phenomenology of Mind as a work on the problem of knowledge. In the first chapter, entitled “Sense Certainty, or the This and Meaning,” he concluded that knowledge cannot consist of an immediate awareness of particulars ). The tradition discusses sense certainty in terms of this failure of immediate knowledge without, however, specifically addressing the problem of reference. Yet reference is distinct from knowledge in the sense that while there can be no knowledge of objects without reference, there (...) may be reference without knowledge. If that is the case, then the failure of immediate knowledge does not entitle us to conclude anything about the success or failure of reference. It is not surprising, then, that a few scholars have begun to examine sense certainty primarily as a thesis about reference. (shrink)

The argument of this paper is part of a general defence of the claim that Bentham's moral theory embodies a utilitarian theory of distributive justice, which is developed in his Civil Law writings. Whereas it is a commonplace of recent revisionist scholarship to argue that J. S. Mill had a developed utilitarian theory of justice, few scholars regard Bentham as having a theory of justice, let alone one that rivals in sophistication that of Mill. Indeed, Gerald J. Postema in his (...) book Bentham and the Common Law Tradition, argues that Bentham had no substantial concern with the concept of justice, and that what analysis of the concept there is in Bentham's thought is unlike the utilitarian theory of justice to be found in chapter five of J. S. Mill's Utilitarianism Although Postema's interpretation is not the only one that will be addressed in this paper, it serves as an important starting point for any rival interpretation of Bentham's ethical theory for two reasons. Firstly, it is the most comprehensive and most penetrating discussion of Bentham's utilitarian theory, drawing as it does on a wide variety of published and unpublished materials written throughout Bentham's career. Secondly, it is interesting in this particular context because the contrast that Postema draws between Bentham's and Mill's theories of justice depends upon a particular reading of Mill's theory of justice and utility which is derived from recent scholarship and which is by no means uncontroversial. As part of the defence of the claim that Bentham had a sophisticated theory of distributive justice, it will be argued in this paper that the contrast drawn between Bentham and Mill does not stand up to careful scrutiny, for insofar as Mill's theory of justice can be consistently defended it is not significantly different from the utilitarian strategy that Bentham employed for incorporating considerations of distributive justice within his theory. This is not to claim that there are not significant differences between the theories of justice of Bentham and J. S. Mill, but it is to claim that whatever technical differences exist between their theories, both writers saw the need to incorporate the concept of justice within utilitarianism. Therefore, rather than showing that Mill is an interesting thinker to the extent that he abandons his early Benthamism, by demonstrating how close Mill's theory of utility and justice is to that of Bentham, it will be possible to argue that Bentham employed a sophisticated and subtle utilitarian theory that was responsive to the sort of problems which occupied Mill a generation later. A core question of contemporary social morality concerns how we ought to handle racial categorization. By this we mean, for instance, classifying or thinking of a person as Black, Korean, Latino, White, etc.² While it is widely FN:2 agreed that racial categorization played a crucial role in past racial oppression, there remains disagreement among philosophers and social theorists about the ideal role for racial categorization in future endeavors. At one extreme of this disagreement are short-term eliminativists who want to do (...) away with racial categorization relatively quickly (e.g. Appiah, 1995; D’Souza, 1996; Muir, 1993; Wasserstrom, 2001/1980; Webster, 1992; Zack, 1993, 2002), typically because they view it as mistaken and oppressive. At the opposite end of the spectrum, long-term conservationists hold that racial identities and communities are beneficial, and that racial categorization —suitably reformed —is essential to fostering them (e.g. Outlaw, 1990, 1995, 1996). While extreme forms of conservationism have fewer proponents in academia than the most radical eliminativist positions, many theorists advocate more moderate positions. In between the two poles, there are many who believe that racial categorization is valuable (and perhaps necessary) given the continued existence of racial inequality and the lingering effects of past racism (e.g. Haslanger, 2000; Mills, 1998; Root, 2000; Shelby, 2002, 2005; Sundstrom, 2002; Taylor, 2004; Young, 1989). Such authors agree on the short-term need for racial categorization in at least some domains, but they often differ with regard to its long-term value. (shrink)

Although John Stuart Mill places considerable emphasis on three information signalling devices – debate, votes and prices – he remains curiously sceptical about the prospects of institutional or social epistemology. In this paper, I explore Mill's modest scepticism about institutional epistemolog y and compare and contrast that with the attitudes of liberal theorists such as F. A. Hayek and John Dewey who are much more enthusiastic about the prospects of social epistemology as part of their defences of liberalism. The paper (...) examines the extent to which Hayek and Dewey ignore concerns originally raised by Mill. I conclude that Mill's modest scepticism is reflected in the epistemological abstinence of contemporary liberal philosophers such as John Rawls, and that his elevation of philosophy over democracy remains a challenge to contemporary defenders of the political value of social or institutional epistemology. A comprehensive and systematic reconstruction of the philosophy of Charles S. Peirce, perhaps America's most far-ranging and original philosopher, which reveals the unity of his complex and influential body of thought. We are still in the early stages of understanding the thought of C. S. Peirce (1839-1914). Although much good work has been done in isolated areas, relatively little considers the Peircean system as a whole. Peirce made it his life's work to construct a scientifically sophisticated and logically rigorous philosophical (...) system, culminating in a realist epistemology and a metaphysical theory ("synechism") that postulates the connectedness of all things in a universal evolutionary process. In The Continuity of Peirce's Thought, Kelly Parker shows how the principle of continuity functions in phenomenology and semeiotics, the two most novel and important of Peirce's philosophical sciences, which mediate between mathematics and metaphysics. Parker argues that Peirce's concept of continuity is the central organizing theme of the entire Peircean philosophical corpus. He explains how Peirce's unique conception of the mathematical continuum shapes the broad sweep of his thought, extending from mathematics to metaphysics and in religion. He thus provides a convenient and useful overview of Peirce's philosophical system, situating it within the history of ideas and mapping interconnections among the diverse areas of Peirce's work. This challenging yet helpful book adopts an innovative approach to achieve the ambitious goal of more fully understanding the interrelationship of all the elements in the entire corpus of Peirce's writings. Given Peirce's importance in fields ranging from philosophy to mathematics to literary and cultural studies, this new book should appeal to all who seek a fuller, unified understanding of the career and overarching contributions of Peirce, one of the key figures in the American philosophical tradition. Dealing with challenging topics like race and gender in the classroom can be a daunting task. Even when we mean well and try hard, we can easily make mistakes that can have serious consequences for our students, especially those in targeted or oppressed groups. Whether or not we explicitly discuss race and gender in our classes, well-meaning professors and students who believe in equality and social justice often commit racist and sexist microaggressions, which are words and actions that, generally unintentionally, (...) convey racist and sexist messages. These microaggressions have a negative impact on students, and impede their learning process. In this paper, I will explain what microaggressions are and why they happen, in order to help prevent them from occurring. I will also examine ways of effectively managing them when they do occur. Josiah Royce (1855-1916) was the leading American proponent of absolute idealism, the metaphysical view (also maintained by G. W. F. Hegel and F. H. Bradley) that all aspects of reality, including those we experience as disconnected or contradictory, are ultimately unified in the thought of a single all-encompassing consciousness. Royce also made original contributions in ethics, philosophy of community, philosophy of religion and logic. His major works include The Religious Aspect of Philosophy (1885), The World and the Individual (1899-1901), The (...) Philosophy of Loyalty (1908), and The Problem of Christianity (1913). Royce's friendly but longstanding dispute with William James, known as "The Battle of the Absolute," deeply influenced both philosophers' thought. In his later works, Royce reconceived his metaphysics as an "absolute pragmatism" grounded in semiotics. This view dispenses with the Absolute Mind of previous idealism and instead characterizes reality as a universe of ideas or signs which occur in a process of being interpreted by an infinite community of minds. These minds, and the community they constitute, may themselves be understood as signs. Royce's ethics, philosophy of community, philosophy of religion, and logic reflect this metaphysical position. Recent political turmoil has focused international attention on Egypt, yet there is little awareness of the country’s stateless populations—those who lack legal nationality to any state—or the challenges they face. Individuals in situations of protracted statelessness are denied their right to a nationality, resulting in an array of additional rights violations. Such violations include denied freedom of movement, equality before the law, and access to economic and social rights. Drawing from two years’ of fieldwork data, this study highlights the plight (...) of those who are unable to achieve legal status in a country with harsh punishments for illegal presence, entry, and exit. It also evaluates potential solutions for eliminating statelessness in Egypt and protecting the rights of stateless populations. As Egypt seeks to move beyond revolution, it is vital that the government addresses the pervasive and systemic inequalities that deny individuals their right to a nationality. (shrink)

This article demonstrates how and why "meditations on first philosophy" is an unsuccessful attempt by rene descartes to reconcile his rationalist philosophy with his apparently conflicting voluntarism and with his adherence to certain theological principles.

This work extends the consideration of spirituality and leadership to the field of strategic leadership. Future development in the field of spirituality and leadership will depend on greater clarity concerning the level of analysis, and will require a distinction between personal and collective spirituality. Toward that end, a framework is proposed that describes how the personal spiritual beliefs of a top level leader operate in strategic decision making like a schema to filter and frame information. This function is mediated by (...) the leader’s constructive development and meta-belief and moderated by the organizational context and leadership style. This framework provides a starting point for considering the many expressions of spirituality in organizations and serves as a foundation for a multi-level theory of spirituality and leadership. The ecohumanist/service-learning approach to environmental education provides a bridge between science and public policy on the one hand, and direct civic action on the other. This pedagogy appears to be a promising way to engage students and to extend the reach of environmental education beyond the classroom. Peter Hare's writings on civil disobedience suggest that he was not a "quiet man," though he was indeed soft-spoken. He was certainly earnest about matters of conscience, about doing the right thing and doing things right. He was a model of intellectual integrity for several generations of American philosophers. Moreover, when he saw a need he seldom hesitated to take it on himself: sitting on many, many dissertation committees, editing a major philosophical journal, helping found new professional associations. My fellow panelists and I are generally searching for what Robert C. Neville calls a "high road around modernism," a road that leads out of the modernist morass while avoiding the pitfalls of Euro-style postmodernism. We seek a way toward genuine community, and toward the kind of meaningful individualism that can exist in such communities. We stake quite a lot on the Roycean model of community as perhaps the most promising path on this "high road." In the next twenty minutes, (...) I propose to do three things.[1] The first is to outline C. S. Peirce's model of the working scientific community, which he proposed as an alternative to the Cartesian-modernist model of scientific investigation. The second is to identify the ways in which Josiah Royce developed and extended Peirce's original model to apply to other communities--particularly religious communities. Finally, I want to draw attention to some possible problems with such extensions of the basic Peircean model of scientific community. (shrink)

This article elaborates on Putnam's ''discrete behavioral states'' model of dissociative identity disorder (Putnam, 1997) by proposing the involvement of the orbitalfrontal cortex in the development of DID and suggesting a potential neurodevelopmental mechanism responsible for the development of multiple representations of self. The proposed ''orbitalfrontal'' model integrates and elaborates on theory and research from four domains: the neurobiology of the orbitalfrontal cortex and its protective inhibitory role in the temporal organization of behavior, the development of emotion regulation, the development (...) of the self, and experience-dependent reorganizing neocortical processes. The hypothesis being proposed is that the experience-dependent maturation of the orbitalfrontal cortex in early abusive environments, characterized by discontinuity in dyadic socioaffective interactions between the infant and the caregiver, may be responsible for a pattern of lateral inhibition between conflicting subsets of self-representations which are normally integrated into a unified self. The basic idea is that the discontinuity in the early caretaking environment is manifested in the discontinuity in the organization of the developing child's self. (shrink)

In this essay, we consider a set of related questions concerning the role and nature of money, the working of markets, and the relationship between forms of social organization and money. Among other things, we speculate that efforts to purge the neo-classical theory of markets of the phenomenon of false trading have been misguided in the sense that they fail to grasp the dependence of a market system on the existence of some false trading.

While Karen Warren offers an ecofeminist ethic that is pluralistic, contextualist, and challenges Cartesian dualism, one area that remains underdeveloped in her theory is embodiment. I will examine Merleau-Ponty’s notion of embodied subjectivity and show that it would fit consistently with her theory. I will also explore some other areas in which the two theories supplement each other.

“I have a hard year, a year of effort before me. . . . I think I shall very soon be completely ruined; it seems inevitable. What I have to do is to peg away and try to do my duty, and starve if necessary. One thing I must make up my mind to clearly. I must earn some money every day” (W8 lxiv).
